By booking a service with Melizn Aesthetics, you agree to the following terms:

1. Client Responsibility & Disclosure

• I affirm that I have disclosed all known allergies, sensitivities, previous chemical services (color, relaxers, perms, keratin, etc.), and treatments performed on my hair, skin, or nails.

• I understand that withholding or failing to disclose relevant information (including past services, at-home treatments, medications, or allergies) may increase the risk of adverse outcomes.

​

• I agree that it is my sole responsibility to inform the service provider of any changes to my health or personal care history before each service.

​

2. Medical Conditions & Contraindications

• I understand that it is my responsibility to consult with a physician before undergoing any beauty or cosmetic service if I have a medical condition or health concern.

​

• I release Melizn Aesthetics, its owner, employees, and contractors from any liability in the event that I experience an adverse reaction due to an undisclosed or known medical condition.

​

• If I knowingly choose to receive a service against professional advice or while having a contraindicating condition (including pregnancy-related restrictions), I do so at my own risk.

3. Waiver of Liability

• I acknowledge that beauty services may involve the use of chemicals, tools, or products that carry inherent risk. While safety and sanitation are top priorities, I release Melizn Aesthetics, its owner, staff, and affiliates from any responsibility for unexpected outcomes, injuries, skin/hair reactions, or other complications arising during or after the service.

• This waiver extends to all current and future appointments.

4. Booking & Cancellation Policy

• I understand that late arrivals, no-shows, and last-minute cancellations may result in fees, denied service, or forfeiture of deposit.

• Deposits are non-refundable unless otherwise stated in writing.

​

5. Photography & Social Media

• I consent to before-and-after photos being taken for documentation and promotional purposes unless I request otherwise in writing prior to the start of the service.

6. Professional Conduct

• Melizn Aesthetics reserves the right to refuse service to anyone for any reason, including but not limited to inappropriate behavior, safety concerns, or violations of salon policy.

By booking a service, I confirm that I have read, understood, and agree to the terms above. I release Melizn Aesthetics and its staff from any and all liability related to the services I receive.
